1. Adjust tension
2. If the harmonic balancer (crank pulley) is starting to separate then the grooves won't be parallel and will eat up belts. Same goes for any other pulleys that could be bent (power steering)
I would lean towards one of those two since it sounds like a gradual degradation is happening. A mechanical failure would likely have symptoms constantly...
